The following information is needed for Confirmation candidates:

Complete Baptismal name:  As listed on your Baptismal certificate

Confirmation name:  A name is not required but are typically chosen.  The name should be of a Saint. The Bishop will ask what name you have chosen and why.  You can "Google"  "Saints" and should be able to come up with a list to choose from and their meanings.

Sponsors:  The sponsor must be a confirmed, practicing Roman Catholic.  If they are not a member of Immaculate Conception, they must obtain a letter from the Pastor of the Parish in which they are registered.  Note:  If your sponsor needs to travel, they do not need to be present at the rehearsal as a "stand in" can be used and the information can be relayed.
